Abstract

|
Presented here a case of lymphomatous meningitis with asymmetrical multiple cranial
nerve palsies.
He was a 30 year old male who came to this hospital because of headache, diplopia,
vomiting, hoarhness and dysphagia which developed subacutely. He had been diagnosed
non-Hodgkin's lymphoma(DLPD), stage Ⅳ B with the primary involved site of left
femur 6 months previously and treated with local irradiation and 6 cycles of BACOP
combination chemotherapy until this admission. Neurological examination showed multiple
cranial nerve dysfunctions including left Ⅲ and Vll nerve palsies, bilateral Vl nerve
palsy and bulbar muscle weakness. The C-T brain scan was normal. The cerebrospinal
fluid study revealed the meningeal inflammation with 1ow glucose content and malignant
cells.
The patient was treated with CNS irradiation and intrathecal methrotrexate with
marked improvements of neurologic deficits.
